Patient: 4 year old male
History: Admitted with decreased conscious level, fever and a seizure. No focal neurology. Previously well.

Findings: CT Soft evidence of patchy white matter low attenuation MRI patchy becoming confluent areas of increased T2 signal, that show minimal mass effect, in the white matter and basal ganglia
Diagnosis: Acute disseminated encephalomyelitis
Discussion: Typical appearance. Ususally the dramatic MRI appearance is out of keeping with, an often mild, clinical presentation. Made an uneventful recovery
