Patient: 17 year old female
History: 17 year old HCG negative female with right adnexal pain

Findings: Grey scale and Doppler sonographic images of a predominantly cystic right adnexal mass with heterogeneous central echoes with septations and debris some of which layers without central Doppler flow.
DDx:

hemorrhagic cyst, endometrioma, dermoid, ovarian torsion, abscess, pyosalpinx, hydrosalpinx, cystic neoplasm

Diagnosis: hemorrhagic cyst
Discussion:

Hemorrhagic cysts can occur after a ruptured follicle rebleeds internally and expands.  They may mimic many other adnexal masses but are predominantly cystic on ultrasound and demonstrate increased through transmission.  Central echoes with strands and septations or blood products are common without evidence of central Doppler flow.  Another pattern can include diffusely increase echotexture centrally with increased through transmission.  Hemorrhagic cysts can be of any size and of varied echogenicity.  Almost all hemorrhagic cysts will resolve within 1-2 menstrual cycles as occurred in this case.
