Patient: 11 day old male
History: complex congenital heart disease with singl atrium and single ventricle.

Findings: The bronchial branching pattern is consistent with 2 left lobes. No minor fissures are seen. The trachea is in the midline, and there is a right aortic arch which descends on the right side. The mediastinum is widened, most likely with 2 superior vena cavas. On the lateral projection the inferior vena cava (IVC) is not definitely seen, questioning an interrupted IVC. There is a horizontal liver. There is a small gas collection in the left medial subdiaphragmatic region, which may represent a stomach bubble. 12 ribs are noted.
Discussion: you would expect polysplenia with left-sided isomerism.
