Fig. 1: Right kidney demonstrates a swollen, hypoechoic lower pole.

Fig. 2: This is even better appreciated by scanning in the prone position.

Fig. 3: There is also patchy mid-kidney swelling and hypoechogenicity.

Fig. 4: There is decreased flow, as shown with power doppler, in the right lower pole.

Fig. 5: This is the same image with power doppler removed.

Fig. 6: There is a similar finding in the upper pole.

Fig. 7: Notice how it is more subtle on the grey-scale image without doppler. Dx = pyelonephritis. Subsequent VCUG showed bilateral Gr. 5 reflux.
