Fig. 1: CT through upper chest shows a large, septated air filled lesion occupying the left hemithorax. Note right lung atelectasis.

Fig. 2: There is a little residual, aerated left upper lobe and lingular region.

Fig. 3: The heart and mediastinal structures are shifted rightward. Non-atelectatic right lung is otherwise normal, therefore this is not a diffuse (ie. bizzare emphysematous) process.

Fig. 4: There is some residual, normal left lower lobe. By now we're fairly sure this air-filled structure is in the lung.
