Fig. 1: STIR shows high signal lesion centered in the epiphysis, extending across to to the metaphysis. There is a dark border and ill-defined high signal extending off into the lateral epiphysis. Note high signal along medial tibal cortex. No effusion.

Fig. 2: T1 shows dark signal at the center of the abnomality consistent with central cystic change.

Fig. 3: T1 show central fatty marrow abnormality. The cortex appears violated. There is intermediate signal both deep and superficial to the crural fascia.

Discussion: 19 mo male w/ left tibial lesion seen on plain film (no longer available) as a lytic lesion w/ sclerotic border involving the medial aspect of the proxoimal tibial epiphysis and adjacent metaphysis.
