Patient: 7 day old male
History: Hypotonic baby diagnosed with Trisomy 21 both clinically and by chromosome studies. Heart murmer.

Findings:

1. Satisfactory position of NG tube

2. Borderline cardiomegaly

3. No plethora

4. 11 pairs of ribs

5. Oxygen tubing

Diagnosis: Trisomy 21, atrial septal defect, 11 pairs of ribs
