Patient: 52 year old male
History: right heel pain

Findings:

1. lucent lesion in calcaneal tuberosity with sclerotic margin
2. increased T2 signal in lesion
3. S/P curettage and bone grafting with evidence of incorporation of bone graft material on CT

DDx: The calcaneus can be considered an epiphyseal equivalent for purposes of DDx. Thus we should include entities such as chondroblastoma and giant cell tumor.
Diagnosis: The usual DDx was not of much help here.
Discussion: Plain radiograph 3 months S/P curettage and bone grafting suggested a zone of lucency around the graft material, raising the question of graft resorption. However, the CT shows that this more lucent zone is actually composed of mature bone, consistent with interval incorporation of the peripheral graft material.
