Patient: 60 year old male
History: pain

Findings:

Ill-defined lytic lesion of proximal metaphysis, extending up into greater tuberosity and humeral head, with loss of part of lateral cortex of metaphysis.

Also has glenohumeral osteoarthritis.

DDx: In this age group, the usual DDx for this type of lesion is metastasis vs. myeloma.

This case is a good reminder to consider entities such as lymphoma and leukemia as well.
Diagnosis: Follicular lymphoma, primary to proximal humerus
