previously visited MESENTERIC INJURY Random Case

Patient: 40 year old male
History: 40 year old male: ,following MVA ,his FAST exam revealed fluid colllection at both Morrison's pouch & pelvic regions,so CT exam of the abdomen & pelvis with IV contrast was the next step.

Findings:

CT exam of the abd& pelvis with IV contrast:

note the minimal perihepatic fluid collection ,right paracolic gutters,interloop& also the pelvic fluid(arrowed)

then you can compare the stranding of the mesentery at the site of the trauma(arrowed) & the other parts of the mesentery (that isn't affected),

also note the bowel loops near the area of the stranding of the mesentery are seen distended with thickened & enhanced wall

This patient was managed coservatively &had uneventful recovery....

 

Diagnosis: mesenteric injury
Discussion:

 

Mesenteric Infiltration

Mesenteric infiltration or “stranding” can be associated with mesenteric injury with or without bowel perforation , but bowel-wall thickening associated with stranding is highly suggestive of significant bowel injury . Sensitivities and specificities of 69%—77% and 44%—100%, respectively, have been reported for this sign . Mesenteric findings are more common when bowel injury is along the mesenteric border .. A localized hematoma within the mesentery in the absence of a bowel abnormality points to an isolated laceration of a mesenteric vessel .
