previously visited 090317 CONGENITAL DIAPHRAGMATIC EVENTRATION Random Case

Patient: 1 year 10 month old male
History:

1 year 10 month old male

Chief complain:
Cough with sputum for days.
Fever (+), Shortness of breath (+).
WBC: 19430/mm3, Seg: 69%

Findings:

Chest plain film radiography (PA and lateral):

1. Marked elevation of air-containing bowel loops into left
   lower chest, nature to be determined.

Multidetector row CT scan of chest and upper abdomen with
 intravenous contrast, and with multiplanar reconstruction:

1. Obvious upward migration of spleen (red arrow in fig 2),
   air-containing bowel loops (yellow arrow in fig 2),
   part of stomach (green arrow in fig 2), and lateral segment
   of left lobe liver (pink arrow in fig 2) into
   left lower chest without detectable focal defect of left
   hemi-diaphragm, compatible with diaphragmatic eventration.
2. Presence of mass effect of above structure with some degree
   compression of adjacent left lower lung.
   Small alveolar densities in left lower lobe lung, may due to
   inflammatory process.

Diagnosis: Congenital diaphragmatic eventration
