previously visited PULMONARY ABSCESS DUE TO BURKHOLDERIA MEILIOIDOSIS Random Case

Patient: 2 year 2 month 18 day old female
History: This is a two years and 2months old girl has been sick for 5 days of high fever,productive cough, dyspnea and poor appetite.She was admitted in our hospital and treated her as consolidation pneumonia of right middle lobe but she was not improved, still spiking fever around 39 degree. Then we repeated chest x-ray showed lung abscess as seen in picture attached and what we found below.

Findings:

1-PA chest x-ray dated on 28th May 2005 showed: consolidation of right lower zone and right heart border was not visualized, suggestive of consolidation of right middle lobe.
2- PA and Lateral chest x-ray projection dated on 7th July 2005 showed: Consolidation of right middle lobe with centrally air fluid level, suggestive of lung abscess.
3-Chest ultrasound showed: right pleural effusion measured 5mm then pleural aspiration under ultrasound guide showed: purulent fluid.
4-Culture fluid from purulent pleural effusion showed: Safty pin suggestive of Burkholderia Pseudomallei (Melioidosis).

Diagnosis: Pulmonary Abscess due to Melioidosis
