previously visited 080415 RETROCEREBELLAR LARGE ARACHNOID CYST Random Case

Patient: 8 month old male
History:

Delayed milestone compared with his twin sister was noted by the parents.

Brain echography: severe bilateral ventriculomegaly, suspicious obstrutive hydrocephalus

Birth history:
Preterm
Gestational age: 29+3/7 weeks
G3P3, twin A, Caesarean section
Apgar score: 1'(4)->5'(6),
Birth body weight: 1228 gm.

Findings:

MRI of brain with T1 weighted echo train spin echo, T2 weighted
 echo train spin echo, T2 weighted fluid-attenuated inversion
 recovery echo train spin echo, MR diffusion imaging, T2 weighted
 short inversion time inversion recovery echo train spin echo, and
 without intravenous contrast:

1. Focal large amount cerebrospinal fluid accumulation in right
   retrocerebellar space with mild across midline, may due to
   large arachnoid cyst (figure 1).
2. Presence of mass effect of the above lesion with compression of
   adjacent cerebellum and medulla oblongata, and suspicious
   foramen of Magendie.
   Downward displacement of cerebellar tonsil (1.3cm below foramen
   magnum), suggesting tonsillar herniation (arrows in figure 2).
3. Marked dilatation of ventricles with stretched corpus callosum,
   possibly associated with severe obstructive hydrocephalus from
   compressed foramen of Magendie.

Impression:
1. Right retrocerebellar large arachnoid cyst, suspicious tonsillar
   herniation and severe obstructive hydrocephalus.
2. Recommendation: neurosurgery consultation. Thank you!

Diagnosis: Arachnoid cyst
Discussion: Progress: The patient received cysto-peritoneal shunt smoothly days later.

Re: Would it be proper to call this a variant Dandy-Walker malformation? Thank you for your comment! I don't think so. Dandy-Walker variant is milder form of Dandy-Walker malformation. There is variable hypoplasia of inferior vermis of the cerebellum in Dandy-Walker variant. Retrocerebellar arachnoid cyst is an arachnoid cyst behind the cerebellum. Arachnoid cyst is a true intraarachnoid cyst. Large arachnoid cyst with mass effect may get benefit from some surgical procedures.
