Findings: MRI of spine (focus on lumbar spine) with T1 weighted echo train
spin echo, T2 weighted echo train spin echo with/without fat
saturation, pulse-gated cine phase contrast flow images, and
without intravenous gadolinium-enhancement:
1. Obvious short T1 fat signal along thick filum terminale with
normal location of conus medullaris at the level of L1 (fig.1)
and decreased spinal cord motion (peak velocity of spinal cord=
0.57cm/sec, cerebrospinal fluid= 2.74cm/sec, background muscle=
0.49 cm/sec respectively) (fig.2), suggesting fibrolipoma of
the filum terminale with tethered cord.
2. Mild-moderate scoliosis along T-spine with multiple lateral
hemivertebrae (fig3).
3. Mild distended bladder, significance unknown.
4. Non-visualization of the midline dimple in sacral region, which
noted by bedside gross observation. No detectable tract of the
dermal sinus from this examination.
Impression:
1. Fibrolipoma of the filum terminale with tethered cord.
2. Multiple hemivertebrae.
3. Recommendation 1: you may arrange bladder ultrasound to check
pre- and post-voiding urine volume, if clinical suspicious
neurogenic bladder.
4. Recommendation 2: Please arrange chest X-ray. Thank you!
