Patient: 25 year old female
History: Vaginal bleeding during first trimester pregnancy

Findings: A gestational sac, a yolk sac, and an embrionic pole are identified. The embrionic pole measures 1.2 cm, correlating with a menstrual age of 7 weeks and 3 days. Embrionic heart activity is documented at 160 bpm. Both ovaries appear normal. The cervix appears normal. No adnexal masses are identified. There is an unusual 3 cm soft tissue mass either inside, or bulging into, the gestational sac. There is some internal cystic change within the mass.
Diagnosis: The mass is of unknown etiology, but may represent a large area of hemorrhage. Differentials include a polyp or placental mass. Follow up of 2-4 weeks was recommended.
Discussion: A follow up exam was performed at 13 weeks and 0 days. The single live intrauterine pregnancy showed appropriate interval growth, and heart activity was documented at 167 bpm. There was no longer evidence of the previously noted mass.
