Patient: 30 year old
History: 30 yo male with abdominal pain x 12 h (additional history withheld)

Findings: Persistent irregular crescent of air outlining the right kidney despite changing position from erect to supine to LLD. No air in renal parenchyma.
Diagnosis: Retroperitoneal air on the right.
Discussion: ER later reported that patient has Gardner's syndrome, and underwent colonoscopy and polypectomy the previous day. The retroperitoneal air on the right is likely from an ascending colon perforation. There were no symptoms of pyelonephritis, nor is air seen in the renal parenchyma.
