Fig. 1: at birth

Fig. 2: 10 months old

Fig. 3: Post operation
Findings:

1. Overgrowth of both soft tissue and osseous structures of left
2nd-5th toes with widened inter-metatarsal spaces and
associated complete soft tissue syndactyly over 2nd-3rd toes,
   in favor of macrodystrophia lipomatosa.
2. Please check other organ elsewhere in the body to exclude
   the less posibility of neurofibromatosis.
3. Recommendation: further evaluation with MRI of feet, if clinical
   necessary.

Diagnosis: macrodystrophia lipomatosa, syndactyly
Discussion: This patient received plastic surgery and pathology revealed fibroadipose tissue with mild congestion, no malignancy
Comments:

Q: How differentiate neurofibromatosis from macrodystrophia lipomatosa?

A: It may be difficult.

Some radiographic fingings may suggest neurofibromatosis:

     1. Bilateral

     2. Severer in proximal digitals than distal

     3. Other organs problem
